Case management guides the journey from the onset of treatment through aftercare recovery programs and often extends beyond, fostering accountability in the recovering individual and initiating the healing process for both the individual and their family.

When families or clients inquire about case management, it encompasses various roles. Primarily, it involves engaging a professional to help manage what has become unmanageable and to provide recovery support.

At Interventions Canada, our team has extensive experience in aftercare treatment. Clients, after completing inpatient treatment programs, grow accustomed to a structured recovery method within a secure environment. Transitioning from inpatient care to community living can be eased through case management, offering accountability and ensuring that both the individual and their family receive support throughout the recovery journey.

Navigating wraparound services for individuals and their families helps manage the complexities of addiction and/or mental health issues. Case management ensures that everyone has the chance to receive professional treatment.

Many individuals opt for family coaching services when they are unable to commit the time required to manage their loved one’s care.
Case management aims to reduce the emotional stress experienced by families and individuals, fostering healthy relationships throughout the recovery journey.

We deliver professional support and guidance with an emphasis on accountability for the individual in recovery, as well as for their family and friends who provide support.

Steps to Engage in Case Management

Consultation

Case management provides a complimentary 30-minute consultation to discuss the present circumstances.

Location

Case management can be conducted both in person and virtually.

Information Package

A case management information packet, including our fee structure, will be emailed to our clients.

Evaluation

Our team will arrange a phone or virtual meeting to assess and evaluate the current situation.

Treatment Plan

A comprehensive treatment plan is available, starting at 30 days and extending further to accommodate the client's needs.

Frequency

Client Engagement and Coaching

Our case managers will engage with clients on a weekly basis, and our family case managers will offer coaching support to family members.
