The course is designed to acquaint participants with the implementation of soft intervention models, crisis management, and case management techniques. It encompasses a collaborative process and ensures a comprehensive grasp of evaluation, assessment, treatment planning, and care coordination. Case management plays a crucial role in steering individuals from the beginning of treatment to aftercare recovery programs and frequently continues thereafter.

Case management is a collaborative process of evaluation, assessment, treatment planning, and care coordination. Case management involves advocating for individuals and their families by providing communication and resources.

Case Management Course Highlights:

  • This training will introduce participants to facilitating soft interventions models, crisis management and case management.
  • Participants will learn the structure of case management.
  • Participants will learn how to create case management treatment plans and pre and post treatment support for families and/or their loved ones.
  • Participants will learn how to assess individuals and families, provide resources and facilitate coaching services.
  • The educational goal of 14 hours training is for the training participants to gain an understanding of intervention models and to perform soft interventions.

  • To gain skills on how to facilitate evaluating individuals and/or families by providing a concrete plan to utilize community resources and /or providing coaching services.
