What is Case Management? Case management provides education and professional accountability for individuals affected by addictions and/or mental health challenges. Our detailed case management minimizes the emotional pressures families and/or individuals experience in order to encourage the rebuilding of healthy relationships through the recovery process.

Specific goals of our case management services include teaching individuals how to live and practice recovery skills, providing tools to help individuals abstain from drugs and/or alcohol, to provide supportive recovery coaching that helps identify triggers the person has and to assist in the development of healthy boundary and goal setting for a brighter future all while ensuring the families and loved ones of the identified person are receiving the specific support they need to rebuild healthy dynamics for the collective wellbeing of all parties involved.

Who can benefit? Case Management is designed to support individuals completing treatment programs as well as families needing professional support, whether that is supporting the family while their loved one enters a treatment program or navigating alternative options should their loved one not accept treatment at the time of the intervention.

 Assessment: The first 30 days of our case management services is focused on the assessment of the case specifics and the initial development of treatment recommendations, options and a plan of action for the individual as well as all those involved in the case.

After the first 30 days, a review of ongoing challenges and needs will take place and continued recommendations and support services will be revisited, with updates to recommended next steps clearly outlined.

Case Management for Families? Individuals and families work with Interventions Canada Case Management Services for the following reasons;

  • They have a loved one who is struggling with drugs, alcohol, and/or mental health issues

  • They have a loved one with a desire to learn new recovery skills
  • They want to be a healthy support system for individuals struggling with addictions and/or mental health
  • An individual struggling with addiction and/or mental health challenges is willing to accept treatment, but needs help finding treatment options
  • An individual struggling with addiction and/or mental health challenges needs assessment and referral services
  • They want to find solutions to address a variety of ongoing crisis interventions
